Unifi Healthcare in Alabama is seeking an RN Case Manager to oversee home-health care planning, implementation, and evaluation. You will serve as the liaison between the agency, patients, and families, ensuring high-quality, compliant care within your service area.

Responsibilities include chart reviews, updating QuickBase records, coordinating treatment plans, and providing patient/family education. You’ll attend MD appointments, collaborate with the team, and support scheduling to maintain

Qualifications

• RN licensure is required and must be current.
• Minimum 3 years of progressively responsible clinical experience.
• Excellent written communication and time-management skills.
• Prior management or leadership experience is preferred.
• Ability to work independently and as part of a team, with multi-tasking skills.
• Professional, courteous, compassionate demeanor with strong customer service.
• Must maintain integrity and discretion in handling patient information.
• Proficient computer and internet skills required.
• Ability to lift 40–50 lbs and perform bending tasks as needed.

Responsabilités

• Review documentation and ensure patient POC is followed.
• Maintain accurate client data in QuickBase and update promptly.
• Coordinate care with Authorization Specialists to develop treatment plans.
• Provide implementation and evaluation of care plans per physician orders.
• Educate clients on benefits, use of DME, medications, and authorizations.
• Manage ongoing caseload and attend MD appointments as needed.
• Build rapport with patient families and ensure satisfaction.
• Collaborate with staffing to schedule workload efficiently.
• Maintain communication with DON, ADONs, and other teams.
• Attend monthly Case Management meetings and staff education programs.
• Ensure adherence to agency policies and regulatory requirements.
