Job summary

BAYADA Home Health Care in Warrenton, VA is seeking an experienced Physical Therapist to provide skilled home health visits to adult and geriatric clients. This role supports a multi-disciplinary team and focuses on delivering high-quality rehabilitative care in the home setting.

The PT will assess, educate, and implement individualized care plans, document findings in a state-of-the-art EMR, and collaborate with nursing to optimize client independence and safety.

Qualifications

• A VA Physical Therapist license is required.
• Minimum of one year of recent PT experience.
• Graduation from a program approved by the American Physical Therapy Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, gait, balance and safety.
• Develop and implement individualized care plans with therapeutic exercises.
• continually assess and revise the care plan with the multidisciplinary team.
• Educate clients and family members on rehabilitative care and safety.
• Accurately document observations and interventions in EMR.
