Job summary

ABL Health Care is seeking an experienced Full-Time Physical Therapist (PT) for Home Health in Fremont. You will evaluate patients, develop and implement care plans, and coordinate with Clinical Director and staff both remotely and in the field.

Responsibilities include providing skilled intermittent care, evaluating patient status, administering treatments, managing the plan of care, educating patients and families, keeping accurate records on a company device, and adhering to policies.

Qualifications

• Active California PT license required.
• Minimum 1 year experience in Home Health or Hospice.
• Current California driver’s license and ability to travel 15–25 miles regularly.

Responsibilities

• Provide, coordinate, and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to the team.
• Develop, implement, and revise the patient’s plan of care (POC).
• Address problems identified in the POC.
• Educate patient/family/caregiver about condition and care.
• Communicate with other disciplines involved in the case.
• Maintain accurate clinical records and submit documentation per policy.
• Comply with company policies and procedures for high-quality care.
