ABL Health Care is seeking an experienced Full-Time Physical Therapist (PT) for Home Health in Fremont. You will evaluate patients, develop and implement care plans, and coordinate with Clinical Director and staff both remotely and in the field.

Responsibilities include providing skilled intermittent care, evaluating patient status, administering treatments, managing the plan of care, educating patients and families, keeping accurate records on a company device, and adhering to policies.

Qualifications

• Active California PT license required.
• Minimum 1 year experience in Home Health or Hospice.
• Current California driver’s license and ability to travel 15–25 miles regularly.

Job Title

Physical Therapist (PT) Home Health – Full Time – Fremont

Position Overview

We are seeking an experienced, self‑driven Full‑Time Physical Therapist (PT) to join our dedicated Home Health team and provide exceptional care throughout the region. You will evaluate patients’ health, create and implement care plans, track their progress, and work closely with the Clinical Director and Clinical Staff both remotely and in the field.

Responsibilities

• Provide, coordinate, and oversee skilled intermittent care to patients, including instruction and education of patients and/or caregivers to promote maximal functional status in the patient’s home.
• Regularly evaluate and re‑evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs or symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed.
• Initiate, develop, implement, and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient’s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current PT license to practice in California – required.
• Experience in Home Health or Hospice care – 1 year required.
• Current California driver’s license.
• Able to travel within a 15–25 mile radius on a regular basis.
