Job summary

ABL Health Care in South San Francisco is seeking a Full Time Physical Therapist (PT) to provide home health services, evaluate patients, create and execute care plans, and monitor progress in patients' homes.

You will work with the Clinical Director and a dedicated team, offering compassionate, organized care and traveling within a 15–25 mile radius to deliver high-quality patient support.

Qualifications

• Current PT License to practice in California
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver's License
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Initiates, develops, implements and revises the patient's plan of care (POC).
• Addresses all problems identified in the POC.
• Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
• Communicates with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
