Job summary

ABL Health Care in Pleasanton, CA is seeking a Full Time Physical Therapist (PT) to join our Home Health team. You will assess patients at home, develop and implement care plans, and monitor progress in collaboration with the Clinical Director and staff.

The ideal candidate holds a California PT license, has at least 1 year in home health or hospice, can travel within a 15–25 mile radius, and is committed to delivering compassionate, high-quality care with support from a knowledgeable back

Qualifications

• Current California PT license required.
• Experience in Home Health or Hospice care preferred (1 year required).
• Willingness to travel within a 15-25 mile radius.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the patient's home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor patient conditions, and report changes to the physician/case manager.
