Job summary

ABL Health Care is seeking a full-time Physical Therapist (PT) to join our Home Health team in Alameda, CA. You will evaluate patients in their homes, develop and implement care plans, and monitor progress, collaborating with Clinical Director and staff in field settings.

The role requires a California PT license, at least 1 year of home health or hospice experience, a valid CA driver’s license, and willingness to travel 15–25 miles regularly. Competitive benefits and company devices provided.

Qualifications

• Current PT license to practice in California.
• Home Health or Hospice care experience: 1 year.
• Valid California driver’s license.
• Ability to travel 15–25 miles regularly.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in the patient’s home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments and monitor patient conditions.
• Initiate and revise the patient’s plan of care.
• Address problems identified in the plan of care.
• Educate patients/family/caregivers about condition and care.
• Communicate with other disciplines involved in the case.
• Maintain accurate clinical records using company device.
• Comply with company policies to ensure high standards of care.
