Job summary

Advocate Health at Home is seeking a full-time Home Health Physical Therapist in the Charlotte, NC area. You will manage a caseload, perform PT services, and coordinate with physicians and a care team to deliver high-quality home-based therapy.

The role requires a DPT, active PT license in the state, and the ability to travel to patients’ homes. Compensation includes a sign-on bonus potential and comprehensive benefits.

Qualifications

• Doctor of Physical Therapy degree required; aligns with entry standards.
• Current state PT license in applicable state or South Carolina when working there; BLS healthcare provider certified.
• 1 year home health or pediatric setting experience preferred.

Responsibilities

• Manage daily patient caseload in home health, with timely care and documentation.
• Collect data from patient assessments via home visits, phone, and EMR.
• Evaluate and plan care with physician, supervise related staff to ensure quality outcomes.
• Coordinate multidisciplinary team to promote safe, effective care and timely services.
• Maintain communication with physician oversight and branch supervision; adjust plans as needed.
