Advocate Health at Home in Lincolnton, NC seeks a licensed Physical Therapist to manage a home health caseload, implement plans of care, and coordinate with physicians and a multidisciplinary team.

The role requires a DPT, active state license, BLS certification, and a valid driver’s license with auto insurance. Travel to patients'' homes is expected; weekends/holidays based on schedule.

Competenze

• DPT required; licensed to practice in the state.
• BLS for Healthcare Providers required (American Heart Association).
• Valid state driver''s license and auto insurance.
• 1 year of pediatric PT experience preferred.
• Pediatric home health experience is a plus.

Mansioni

• Manage daily patient caseload and document care per plan of care and MD orders.
• Collect data from home assessments and EMR to guide interventions.
• Develop and implement the plan of care with physician collaboration.
• Coordinate with multidisciplinary clinical team and supervise junior staff.
• Ensure care aligns with regulatory standards and agency policies.
