Job summary

Peace and Harmony Homecare LLC in Boston, MA seeks a licensed Physical Therapist to deliver professional PT services in patients’ homes, within standard care guidelines and in collaboration with primary care physicians and care teams.

You will assess patients, develop and implementPlan of Care, and educate patients and families on safety, devices, and activities of daily living. Travel to patient residences is required; strong communication is essential.

Qualifications

  • Graduate of a CAPTE-approved PT program or equivalent.
  • Active PT license to practice in the United States.
  • Minimum of 1 year experience as a Physical Therapist.
  • Criminal background check and current CPR certification.
  • If trained outside the United States before 2008, meets WCPT/World Confederation criteria.

Responsibilities

  • Receive intake referral information and prepare PT paperwork/tools for visits.
  • Obtain medical history from patient, family, or significant others.
  • Perform PT assessment and document findings.
  • Perform home safety check and environment assessment.
  • Assist physician in evaluating function and develop the PT Plan of Care to be submitted for approval, updating at least every 30 days.
  • Functionally reassess patient for ongoing therapy and determine medical necessity.
  • Evaluate patient ADL abilities and progress.
  • Develops and implements the home health Aide plan of care when PT is the sole skilled discipline.
  • Supervise the Aide as per state/federal requirements and document supervision.
  • Communicate with patient/family about POC and progression; educate on devices as needed.
  • Communicate with physician for treatment modalities and rehab options.
  • Assist physician in evaluating patients using diagnostic and prognostic tests.
  • Submit all OASIS assessments within 48 hours of visit.
  • Submit CRR timely, participate in committee processes and staff meetings.
  • Mentor newly hired staff and contribute to therapy services planning and evaluation.
  • Maintain licensure and pursue ongoing education.
