Job summary

Baptist Home Care in Mississippi is seeking a dedicated Physical Therapist to assess patients’ mobility, endurance, pain and functional status in home health settings. You will develop and implement treatment plans and monitor progress to improve well-being and independence.

You will educate patients and families, perform visits per plan of treatment, document outcomes, and collaborate with the health care team to ensure safe, effective care within state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification (or ability to obtain within 90 days of hire).

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care; works in diverse home environments.
