• PT licensure or eligibility as applicable in the state.

Responsibilities

• Evaluates and assesses the patient based on subjective and objective information.
• Develops a written plan of care.
• Provides treatment according to established goals on plan of care.
• Documents interventions/care in the patient's medical record.
• Evaluates effectiveness of plan of care while reassessing patient’s functional status.
• Provides discharge planning information including patient/family education, follow-up, and referrals.
• Contributes to staff and student development.
• Participates in activities to improve health care delivery.
• Completes assigned goals.
