Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patients, families, and caregivers about plan of care and progress toward goals.
• Completes progress notes after visits and submits them per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
