Job summary

Mountaineer Home Care, part of the Optum family, is seeking a Physical Therapist for Home Health in Kanawha County. You will assess patient needs related to mobility and function, develop a treatment plan, and implement interventions to improve well-being while monitoring progress.

Responsibilities include evaluating functional status, delivering therapy within state scope, documenting visits and progress notes, educating patients and families, and collaborating with the care team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in developing the plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care, coordinating with other health care team members.
• Observes and reports patient responses to treatment and any changes to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maximize functioning according to standards of practice.
• Instructs and informs patient and family regarding plan of care, limitations, and progress toward goals.
• Visits per Plan of Treatment; completes progress notes and submits per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes, goals, and projected frequency of care.
• Function in various home settings regardless of patient demographics.
