Job summary

Mountaineer Home Care, part of the LHC Group family, is seeking a Physical Therapist for home health. You will assess functional status, plan and deliver PT interventions, and monitor patient progress in collaboration with the care team.

You will provide education to patients and families, document visits, and ensure care aligns with state guidelines and the plan of treatment. This role emphasizes compassionate, patient-centered care across diverse home environments.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Valid driver’s license and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient functional status and therapy needs, assisting in plan of care development.
• Provides PT services within scope of practice and in coordination with the health care team.
• Observes and reports patient responses to treatment and any changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops PT plan within state guidelines.
• Reports outcomes and projected frequency of care.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
