UnitedHealth Group is seeking a Physical Therapist to perform assessments and develop treatment plans for home health patients in Kanawha County, WV. You will evaluate functional status, mobility, endurance, and pain, then implement interventions to improve well-being and progress toward goals.

Responsibilities include delivering care per state laws, coordinating with the health care team, documenting visits, and communicating progress to the physician.

Competenze

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Mansioni

• Evaluates patient functional status and PT needs; assists in developing therapy plan.
• Provides services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervisor/physician.
• Treats patients to relieve pain and restore or maintain function.
• Instructs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes PT evaluations and develops plan of care within guidelines.
• Reports outcomes and frequency of care.
• Function in diverse home environments, respecting all patients.
