Qualifikationen

• Current driver's license and access to a dependable vehicle or public transportation.

• Evaluates functional status and CMS documentation needs; consults with the physician in developing the plan of care.
• Observes, records, and reports patient responses to treatment and changes in condition to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using standard tests and measures.
• Function in home settings with diverse patient populations regardless of age, race, or disability.
