Job summary

Concierge Home Care in Vero Beach, FL is seeking a Home Health Occupational Therapist to deliver 1:1 in-home therapy with flexible scheduling and autonomy. You will evaluate patients, develop individualized treatment plans, and support ADLs and functional mobility at home, with Medicare differential pay.

Emphasis on efficient documentation using AI-assisted charting and WellSky/Kinnser EMR, plus mentorship, ongoing training, and career advancement opportunities.

Qualifications

• Active Florida Occupational Therapist license (Florida OT).
• CPR certification valid for clinical settings.
• Valid driver’s license with reliable transportation.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, functional mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Certified Occupational Therapy Assistants (COTAs).
• Complete documentation using streamlined technology.
