Job summary

Mederi Caretenders, part of the Optum family, is seeking an Occupational Therapist for Home Health in Florida. You will assess patient care needs related to functional status, activities of daily living, and adaptive equipment within the plan of care.

Primary responsibilities include evaluating functional status, coordinating with physicians, and documenting patient progress. This role requires licensure, driving access, and strong communication skills to deliver high-quality care.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Good communication, writing, and organizational skills.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.

Responsibilities

• Provides service within the scope of practice as defined by state laws and the plan of care.
• Evaluates the patient’s functional status and coordinates with physician on the therapy plan of care.
• Observes, records, and reports patient response to treatment and changes in condition.
• Conducts patient assessments evaluating level of function with diagnostic tests.
• Function in home settings regardless of age, race, creed, color, sex, or disability.
