Job summary

Join Mederi Caretenders, part of LHC Group, a leading post-acute care partner, and help patients in their homes across Florida. As an Occupational Therapist in Home Health, you will assess and plan care, address ADLs, fine motor skills, and adaptive equipment in coordination with the health care team.

You will be responsible for evaluating functional status, documenting CMS-relevant data, and communicating findings to physicians.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other health care team members.
