Job summary

Mederi Caretenders Home Health is hiring a full-time Occupational Therapist to join our team in Vero Beach, Florida. You will assess patients, evaluate functional status, and address ADLs, fine motor skills, and home-based needs in collaboration with physicians and the care team.

Responsibilities include documenting CMS-compliant data, developing plans of care, and delivering evidence-based therapy in home settings.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
