Job summary

VitalCaring Group is seeking a licensed Occupational Therapist to join our home health team in Oklahoma. You will evaluate patients in their homes, develop goals, and deliver personalized, high-impact care.

This full-time, field-based role emphasizes collaboration with families and interdisciplinary teams to restore function and independence, including patient education, safety assessments, and discharge planning.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience; home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and home safety.
• Develop individualized treatment plans to improve independence.
• Provide therapeutic interventions to restore function.
• Fabricate splints and train in adaptive devices.
• Support patient return to work via evaluations.
• Educate patients and families on plans and safety.
• Collaborate with care team for seamless care.
• Participate in discharge planning.
• Supervise COTAs and oversee home health aide when needed.
