VitalCaring Group in Oklahoma is seeking a compassionate Occupational Therapist to deliver high-quality home health services. You will evaluate patients' function, create personalized treatment plans, and work with families and the care team to restore independence in daily living activities.

This full-time, field-based role emphasizes safety, patient-centered care, and collaboration, with opportunities to mentor staff, coordinate care, and support successful discharges.

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current NBCOT certification
• Current state OT licensure
• Minimum 1 year clinical experience; home health a plus
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate patients' functional status and daily living activities
• Assess home safety and recommend modifications or adaptive equipment
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to restore function
• Supervise COTAs and oversee home health aide care when needed
• Coordinate care with physicians and interdisciplinary team
• Educate patients and families on plans and safety
• Discharge planning and transitions
