Job summary

Aveanna Healthcare is seeking a part-time PRN Occupational Therapist to join our home health team in the Grand Rapids area. You will assess, teach, and deliver OT interventions to patients under physician direction, coordinating with an interdisciplinary team to optimize patient well-being.

Responsibilities include initial and ongoing mobility assessments, developing goals and plans of care, delivering therapy per orders, documenting progress, and contributing to discharge planning and caregiver

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice
• Valid CPR

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility and current level of function.
• Deliver OT services to comply with physician orders and plan of care.
• Maintain plan of care with timely documentation and adjust goals as needed.
• Discharge planning and home exercise guidance for patients and caregivers.
• Collaborate with health care team and educate patients and families.
• Supervise OTA/HHA as applicable and complete OASIS assessments where allowed.
