Job summary

CenterWell Home Health in Sandy Springs, GA seeks an Occupational Therapist to deliver in-home rehabilitation, assess daily living skills and develop retraining plans to restore independence and function.

You will collaborate with physicians and the care team, document progress and discharge plans, and educate patients and families to support ongoing safety and quality of life at home.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs.
• Confer with physician and care team to update the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination and ROM.
• Document patient progress and outcomes; coordinate care and discharge planning.
