Job summary

CenterWell Home Health seeks an Occupational Therapist to deliver in-home rehabilitation, helping patients regain strength and independence. You will assess functional abilities, plan retraining, and work with physicians to optimize the care plan, documenting progress for ongoing care decisions.

Requirements include current OT licensure, CPR certification, and reliable transportation. The role involves coordinating with a multidisciplinary team and contributing to discharge planning.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license with auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient daily living/work-related skills and retrain with measurable objectives.
• Collaborate with physicians and care team to develop the Plan of Care.
• Provide therapy to improve strength, ROM and function per physician orders.
• Document patient progress and session notes accurately and timely.
• Coordinate care and discharge planning with the interdisciplinary team.
