Qualifications

• OT licensure required and active in state
• CPR certification required
• Valid driver's license and reliable transportation

Responsibilities

• Assess daily living/work-related skills and develop retraining programs.
• Collaborate with physicians and care team on Plan of Care.
• Provide therapeutic treatment to improve strength, ROM and function.
• Document care observations and treatment plans.
• Supervise OT Assistant and Home Health Aide performance.
• Design and implement community reintegration and equipment adaptations.

Current and unrestricted OT licensure
