CenterWell Home Health is seeking a licensed Occupational Therapist to deliver patient-centered home-based rehabilitation services. You will assess daily living skills, create retraining plans and work with physicians to optimize care plans, while documenting outcomes and coordinating with the care team.

The role involves home visits, 40-hour work weeks, and collaboration with a multidisciplinary team to support patients in regaining independence and improving quality of life.

Qualifications

• Current and unrestricted OT licensure
• Current CPR certification
• Valid driver's license with reliable transportation

Responsabilités

• Assess/screen patients' daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physicians and health care team members to develop or revise the Plan of Care.
• Provide therapeutic treatment and instruction to improve strength, coordination, ROM and function.
• Review and expand retraining programs to align with rehabilitation goals.
• Consult with vocational team members to create vocationally oriented plans.
• Document patient status, progress and treatment plans.
• Participate in care coordination and discharge planning.
• Design community reintegration activities and coordinate with the interdisciplinary team.
• Recommend adaptive equipment to improve home/work environments.
• Train staff and family on retraining programs and activities.
• Supervise OT Assistant and Home Health Aide performance.
