Job summary

Qualifications

Responsibilities

• Provides skilled occupational therapy in a patient’s home as ordered by the attending physician.
• Evaluates the patient’s level of functioning and prognosis.
• Develops and adjusts an individualized plan of care with changes in needs.
• Educates patients and families on disease/injury management and home exercise programs.
• Documents treatment, assessments and progress per Medicare/Federal guidelines.
• Coordinates with COTAs and care team to ensure continuity of care.
