Job summary

Always Best Care of Greater Cleveland is seeking an Occupational Therapist to deliver home-based therapy across Ohio. You will evaluate patients, develop personalized treatment plans, and set realistic goals for meaningful daily activities within living environments.

Travel to patients' homes is required. The role emphasizes compassionate care, teamwork, and strong documentation. Must hold an Ohio OT license and CPR certification, and be able to work independently.

Qualifications

• Licensed OT in Ohio is required.
• OT program from an accredited school (MS/OTD or BS/equivalent) is required.
• Current CPR certification is required.
• Excellent communication and teamwork skills are essential.
• Ability to work independently and manage a caseload.
• Organized, dependable, and compassionate with patients.

Responsibilities

• Evaluate patients and create personalized treatment plans.
• Set meaningful short- and long-term therapy goals.
• Provide therapy in patients' home environments.
• Coordinate with patients, families, caregivers, and the care team.
• Recommend adaptive equipment when needed.
• Track progress and adjust plans as patients improve.
• Complete accurate and timely documentation.
• Educate patients and caregivers and staff.
• Supervise and support OT assistants as needed.
• Help patients become more independent in daily activities.
