Qualifications

• Hold a Bachelor’s Degree or higher in Occupational Therapy from an accredited school of occupational therapy.
• Be a licensed Occupational Therapist in the practicing state.
• Maintain current CPR certification.
• Demonstrate effective written and oral communication skills and good interpersonal skills.
• Maintain a valid driver’s license and reliable transportation.
• Pass a criminal background check before hire.
• Adhere to ethical guidelines as outlined by the American Occupational Therapy Association.
• Keep current health certificate on file.
• Follow safety standards and body mechanics to protect self and patients.
• Adhere to Harmony policies and dress code; maintain professional appearance.

Responsibilities

• Provide treatment as ordered and approved by the attending physician.
• Evaluate home environment and make recommendations.
• Document patient progress and communicate with the physician and care team.
• Perform initial and ongoing assessments to determine patient functioning.
• Develop and revise the plan of care in coordination with the physician and home health team.
• Supervise aides when applicable and participate in continuing education and team meetings.
