Job summary

Harmony Home Health & Hospice is seeking a Home Health Physical Therapist to evaluate patients in their residences and implement personalized care plans. You will collaborate with care teams to maximize functional outcomes and educate patients and families on exercises and home-use equipment.

The role requires performing home assessments, coordinating care with interdisciplinary teams, and maintaining timely EMR documentation across diverse patient populations in the Albuquerque area.

Qualifications

• Active Physical Therapist license in good standing.
• CPR with BLS certification required.
• Valid driver's license and auto insurance.
• Understanding of federal, state, and local home health regulations.

Responsibilities

• Perform physical and functional patient assessments.
• Develop a plan of care with patient goals.
• Provide evidence-based direct intervention in patients' homes.
• Submit documentation timely.
