Qualifications

• Bachelor’s Degree or higher from an accredited school of Physical Therapy.
• Licensed as a Physical Therapist in the practicing state.
• Prior home health experience preferred.
• Excellent communication and time-management skills.

Responsibilities

• Provide therapeutic treatments and evaluation of equipment needs.
• Develop and revise the plan of care with the physician and other care-team members.
• Evaluate the home environment and make recommendations for safety or other adaptations.
• Conduct initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
• Establish plan of care and goals based on referral and evaluation of functional skills; joint mobility; gait; muscle strength and coordination; sensation tests.
• Implement therapy according to the established plan with appropriate procedures and equipment.
• Educate the patient and/or caregiver about findings, goals, and home programs.
