• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans in line with physician orders.
• Develop home exercise programs with patients and caregivers.
• Coordinate care with other providers and disciplines within the patient’s medical neighborhood.
• Meet weekly minimum patient visits as required by the agency.
• Maintain patient charts, progress notes, and call logs timely.
• Collaborate with agency personnel to align expectations and standards for high-quality care.
• Ensure compliance with agency policies, procedures, and applicable laws.
