Job summary

The Just One Project seeks a Permanent Supportive Housing Case Manager to help individuals and families living with HIV/AIDS find and maintain safe, stable housing in Southern Nevada. You will assess needs, develop tailored service plans, and navigate TBRA alongside healthcare and community partners.

This role blends client meetings, housing coordination, and documentation. A compassionate, confidential approach is essential as you support clients toward independence and improved health outcomes.

Qualifications

• Bachelor's degree in Social Work, Human Services, or related field required.
• 3+ years of experience in case management, social services, or community outreach.
• Experience working with vulnerable or at-risk populations.

Responsibilities

• Provide individualized case management services to clients, helping them identify barriers and set goals for housing stability.
• Conduct comprehensive client assessments and develop personalized service plans.
• Administer TBRA and coordinate rental assistance with landlords and service providers.
• Maintain confidential client records and document services per guidelines.

Education

Bachelor's degree in Social Work, Human Services, or related field
