Qualifications
- Social service or nursing degree from an accredited university program.
- BSW or MSW in social work preferred.
- Minimum of one year’s experience in a service delivery setting.
- Professional experience with EMR documentation preferred.
- Spanish fluency is highly preferred.
Responsibilities
- Learn and remain current on HIV/AIDS and related medical case management.
- Provide medical case management services to identified patients per guidelines.
- Complete biopsychosocial assessments with new and returning patients.
- Manage an average caseload size of 70 patients with ISP development and monitoring.
- Coordinate referrals for medical and non-medical services.
- Act as part of an interdisciplinary care team to assess patient needs.
