Job summary

Positive Impact Health Centers, Inc. (PIHC) seeks a Medical Case Manager to deliver structured case management for adults living with HIV in a clinic setting.

You will work with an interdisciplinary team including medical providers, behavioral health, and patient navigators to optimize patient outcomes. Responsibilities include developing ISP plans, coordinating services, and assisting with eligibility for medication access programs while maintaining HIPAA compliance.

Qualifications

• Social service or nursing degree required, MSW preferred.
• Experience coordinating services for adults with HIV is preferred.
• Spanish language ability preferred.

Responsibilities

• Provide medical case management to eligible adult clinic patients per HRSA/HRSA guidelines.
• Develop comprehensive Individualized Service Plans (ISP) and monitor progress.
• Coordinate referrals for medical and non-medical services; ensure HIPAA compliance.
• Document patient information accurately in EMR and reporting software.
• Participate in an interdisciplinary care team to assess patient needs and coordinate referrals.
