Elderwood at Burlington in Burlington, VT is seeking a Full-Time Physical Therapist to join our rehabilitation team and help residents regain strength, mobility, and independence. The PT provides skilled therapy, develops care plans, collaborates with an interdisciplinary team, and ensures documentation and patient education to support meaningful outcomes.

Licensed PT with Vermont credentials preferred; strong communication and clinical skills required.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current Vermont Physical Therapy license (or eligibility).
• Strong clinical, communication, and documentation skills.
• Passion for working with geriatric and post-acute populations.

Responsibilities

• Evaluate and develop individualized treatment plans for residents
• Deliver skilled physical therapy services per physician orders
• Collaborate with interdisciplinary team to optimize care plans
• Monitor, document, and adjust treatments based on resident progress
• Educate residents and families on therapy goals and outcomes
