BAYADA Home Health Care in Williston, VT is seeking an experienced Physical Therapist (PT) for a full-time outpatient visiting role serving adult and geriatric clients across Windsor County.

The ideal candidate has a Vermont PT license and at least one year of clinical PT experience, is able to work independently, and is proficient with EMR systems. You will join a multidisciplinary team delivering rehabilitative care and helping clients stay safely at home.

Formación

• Vermont PT license required.
• Minimum of one year of PT experience.
• Graduation from an APTA/AMA-accredited PT program.
• Ability to work independently.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make outpatient visits to clients in designated geographic territories in Williston and Windsor County.
• Perform diagnostic tests and measures (mobility, gait, strength, balance, safety).
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Continually assess and revise the PT care plan with the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions using a tablet-based EMR.
