Job summary

Premier Rehab & Healthcare at Burlington is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Burlington, Vermont. You will deliver patient-centered evaluations and individualized treatment plans to promote mobility, independence and safety.

Qualified candidates hold a current Vermont PT license and have strong clinical and communication skills. Experience in SNF or long-term care is preferred, and Net Health experience is a plus.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and Vermont Physical Therapist license.
• Strong clinical, communication, organizational and interpersonal skills.
• Experience in a skilled nursing facility or long-term care setting preferred.
• Familiarity with Net Health documentation.

Responsibilities

• Perform comprehensive physical therapy evaluations and interpret results.
• Develop individualized treatment plans based on orders and goals.
• Provide interventions to improve mobility, strength, balance, endurance and functional independence.
• Educate patients, families and caregivers on treatment plans and safety.
• Document evaluations, treatments, progress and discharge planning.
• Collaborate with physicians and the interdisciplinary team to ensure quality care.
