Physical Therapist (PT)

Premier Rehab & Healthcare at Burlington has an exciting opportunity for a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Burlington, Vermont.

We are seeking a dedicated and compassionate Physical Therapist to provide high-quality, person-centered rehabilitation services to our residents and patients. You will work alongside an experienced clinical and interdisciplinary team to develop individualized treatment programs that promote mobility, independence, safety, and quality of life.

Pay: $42–$52 per hour

Physical Therapist Responsibilities

• Perform comprehensive physical therapy evaluations and interpret evaluation findings and test results.

• Develop individualized treatment plans based on physician orders, clinical findings, patient needs, and established goals.

• Provide skilled physical therapy interventions designed to improve mobility, strength, balance, coordination, endurance, and functional independence.

• Create personalized rehabilitation programs to help patients achieve their functional goals and maximize independence.

• Provide medically necessary physical therapy treatments in accordance with the established plan of care.

• Monitor patient responses to treatment and modify treatment approaches as clinically appropriate.

• Establish and update individualized goals based on patient progress and clinical findings.

• Educate patients, residents, families, and caregivers on treatment plans, therapeutic exercises, safety, mobility strategies, and techniques to promote independence.

• Collaborate with physicians, nursing staff, therapy team members, and other members of the interdisciplinary team to promote coordinated, high-quality patient care.

• Communicate changes in patient condition, progress, and treatment needs to appropriate members of the healthcare team.

• Complete accurate and timely documentation of evaluations, treatment sessions, progress, goals, and discharge planning.

• Maintain compliance with applicable regulations, professional standards, facility policies, and procedures.

• Attend required meetings and participate in other duties as assigned by the Director of Rehabilitation.

Physical Therapist Qualifications

• Graduate of an accredited Physical Therapy program.

• Current and valid Physical Therapist license in Vermont.

• Must be in good standing with applicable licensing and regulatory agencies.

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Strong knowledge of physical therapy treatment practices, techniques, and rehabilitation interventions.

• Strong clinical, communication, organizational, and interpersonal skills.

• Ability to work effectively with residents and patients with a variety of needs and abilities.

• Commitment to providing compassionate, patient-centered care.

• Familiarity with electronic therapy documentation systems, including Net Health, preferred.
