Job summary

Elderwood at Burlington is seeking a compassionate Physical Therapist to join our rehabilitation team across skilled nursing communities. This Full Time & Per Diem PT role focuses on quality outcomes, individualized treatment plans, and meaningful resident progress—not just productivity metrics.

The PT will evaluate and treat residents, develop care plans, document outcomes, and collaborate with interdisciplinary teams to support transitions from hospital to home while maximizing independence.

Qualifications

• Graduation from an approved school of physical therapy.
• State license to practice physical therapy.
• Two years’ experience preferred.
• Ability to direct others.
• English language proficiency required.

Responsibilities

• Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized care plans and therapy goals.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary clinical teams regarding resident progress and needs.
• Participate in resident care planning meetings.
• Assist with education and training initiatives as needed.
• Support a safe, resident-centered care environment.
