Job summary

Arrow Child & Family Ministries is seeking a Case Manager responsible for the treatment and coordination of services for foster/adopt children and their families. This role involves developing assessments, treatment plans, and aftercare services in collaboration with foster families and the treatment team.

The position requires a Bachelor’s Degree in Social Work or related field with at least one year of experience, with a Master’s Degree preferred.

Qualifications

• Bachelor’s Degree in Social Work, Social Services, or related field with at least 1 year of experience.
• Master’s Degree preferred.

Responsibilities

• Facilitate placement of children in foster/adoptive homes and ensure intake paperwork is signed and entered into the system within policy timeframes.
• Participate in development of assessment, treatment planning, service delivery, discharge, and aftercare as determined by child’s service package.
• Assist and monitor families in licensing/regulatory standard compliance to maintain service package requirements.
• Implement Pre-Admission Assessment and Individual Treatment Plans with foster parents, clients, and treatment team.
• Provide consultation for foster/adopt parents, biological parents, respite providers, childcare staff, and other caregivers.
• Maintain accurate case notes, billing documentation, treatment plans, progress notes, and other documentation per standards.
• Attend trainings and meetings; serve as education liaison with referral agency and foster parents.
• Coordinate care of the child with medical consenter and participate in STAR Health Coordination in TX.
• Complete discharge summaries and provide after-care services as required by youth’s service package.
• Maintain child and/or family records in Arrow’s electronic client information system and use Arrow technology resources.

Education

Bachelor’s Degree in Social Work or related field

Master’s Degree preferred
