Job summary

Arrow Child & Family Ministries in Beaumont, TX seeks a Case Manager to coordinate treatment and services for foster/adopt children and their families. You will work within the agency’s framework to ensure timely intake, service planning, and cross-system collaboration with schools and caregivers.

The role emphasizes providing crisis intervention, documentation accuracy, and education liaison duties while maintaining compliance with licensing and agency standards.

Qualifications

• Bachelor’s Degree in Social Work, Social Services, or related field.
• At least 1 year of experience working with children and families.
• Master’s Degree preferred.

Responsibilities

• Facilitate placement of children in foster/adoptive homes and ensure intake forms are completed.
• Develop assessments, treatment planning, service delivery, and discharge planning.
• Assist families with licensing standards and credentialing.
• Provide consultation for foster/adoptive parents, biological parents, respite providers, and other caregivers.
• Maintain accurate case notes, billing documentation, treatment plans, and progress notes.
• Attend trainings and meetings; serve as the education liaison with schools and caregivers.
• Coordinate care with the medical consenter and participate in STAR Health Coordination in TX.
• Complete discharge summaries and provide after-care services.

Education

Bachelor’s Degree in Social Work, Social Services, or related field

Master’s Degree preferred
