Job summary

Arrow Child & Family Ministries in Texas seeks a Case Manager to coordinate services for foster/adopt children and their families, overseeing intake, assessment, and documentation per agency procedures.

You will develop treatment plans, monitor licensing standards, provide crisis intervention on a 24-hour on-call basis, collaborate with schools, clinicians, and caregivers, and maintain detailed records.

Qualifications

• Bachelor’s Degree in Social Work, Social Services, or a related field.
• At least 1 year of experience working with children and families.
• Master’s Degree preferred.

Responsibilities

• Facilitate placement of children in foster/adoptive homes and ensure intake paperwork is signed and entered into the electronic case management system.
• Participate in development of assessment, treatment planning, service delivery, discharge, and aftercare per youth’s service package.
• Assist families in meeting licensing/regulatory standards and credentialing.
• Implement Pre-Admission Assessment and Individual Treatment Plans with foster parents and clients.
• Provide consultation for foster families, biological parents, respite provider, childcare staff, and other caregivers.
• Maintain accurate case notes, billing docs, treatment plans, and progress notes per standards.
• Attend trainings and meetings as required.
• Serve as education liaison with referral agencies and foster parents.
• Participate in case planning conferences, school meetings, court hearings, and cross-system collaboration.
• Maintain regular contact with family and child to monitor well-being and provide crisis intervention as needed.
• Coordinate care with medical consenters and STAR Health coordination in TX.
