Job summary

Necco seeks a Foster Care Case Manager to work with children and foster families to achieve permanency and growth. The role covers placement, intake, assessment, treatment planning, and ongoing support across office, community, school, and home settings.

The position requires strong organizational, documentation, and collaborative skills, plus the ability to navigate multiple software systems and comply with regulatory requirements. Travel and on-call duties are expected.

Qualifications

• Minimum of 21 years of age.
• Current LSW or LPC or Bachelor's degree in a human services-related field (OH, KY, GA).
• Bachelor's degree with field experience of 2 years (WV).
• Bachelor's degree or Master's degree from an accredited four-year college or university (Tennessee).
• The degree must be supplemented by at least 7 hours in behavioral sciences, such as social work, psychology, and sociology.
• One year of casework experience can be substituted for the 7 hours.
• Minimum of 2 years-experience working with children with emotional or behavioral problems.
• Valid Driver’s License
• Limited Liability Auto Insurance Coverage of 100/300/100
• Organizational, and Written/Oral Communications skills
• Must have the skills necessary to navigate and execute several software’s (i.e. Microsoft offices, Databases, Electronic Health Records, Dashboards, etc.)
• Successful completion of all required Criminal Background checks and drug screenings per state and company regulations
• Ability to adhere to scheduled and unplanned deadlines
• Willingness to travel and attend training
• Successful completion of all required training pertaining to job

Responsibilities

• Participate in referral matching for children and families, advocating for best possible home match.
• Develop each child’s Initial Treatment Plan and Comprehensive Treatment Plan.
• Facilitate and/or ensure completion of appropriate assessments (Biopsychosocial, Independent Living, Diagnostic, etc.).
• Refer foster child to applicable support services (behavioral health, independent living, etc.).
• Ensure initial legal documentation is obtained prior to placement or service delivery.
• Ensure safety plans are developed during intake.
• Develop and facilitate each consumer’s Individualized Treatment Plan (ITP).
• Assist foster parents and child with implementation and monitoring of the plan.
• Establish relationships with foster parents and children through training and support.
• Meet and exceed state/licensure regulations regarding contact with foster children.
• Ensure medical and therapy services are provided as outlined in the service plan.
• Serve as liaison between Necco and placement agencies and community resources.
• Participate in service meetings with various stakeholders.
• Establish relationships with school personnel to meet education goals.
• Provide ongoing training and support to foster parents and children.
• Monitor and review documentation to meet state, licensure and accreditation requirements.
• Maintain confidentiality as outlined in policy.
• Provide input for Corrective Action Plans and assist with tasks.
• Monitor and review medication logs to ensure proper administration.
• Ensure discharge planning includes referral to aftercare services.
• Complete home visits as required by rules.
• Deliver services across multiple environments and travel between locations as needed.
• Participate in weekly on-call rotation for advisory and support.
