Job summary

Buffalo Urban League is seeking a Case Planner - Preventive Services to join our Buffalo, NY team. The role provides weekly home-based intervention services to families at risk, overseeing 12 cases, and coordinating with Erie County Department of Social Services.

The incumbent will document progress and collaborate with partners to ensure effective service delivery. A BA/BS in human services and experience in case management are preferred, along with strong communication skills, independence,

Qualifications

• Minimum BA/BS degree preferred in human services or related field, and at least one year of case management experience.
• Working knowledge of computers and NYS CONNECTIONS is a plus.
• Effective verbal and written communication skills.
• Ability to carry out assignments without direct supervision.
• Typing and computer skills; Spanish speaking a plus.
• Valid driver's license, car insurance, and daily access to a vehicle.
• Ability to work flexible hours.

Responsibilities

• Provide mandated preventive services, outreach, case planning, assessment, counseling, and face-to-face home visits.
• Participates in case conferences, service plan reviews, staff training, and court reports.
• Assist clients in developing measurable goals and individual service plans.
• Monitor client progress and provide direct assistance to ensure intervention compliance.
• Advocacy with schools, courts, ECDSS, and other community resources and stakeholders.
• Will have access to the company laptop and cell phone.
