Job summary

Forestdale Inc. in New York City seeks a Family Support Case Planner to work with families at risk, applying Solution-Based Casework to strengthen parenting and family resilience.

You will engage families in their homes or community settings and coordinate services through state systems. The role requires a relevant degree, 1+ year in case management and strong communication skills, with bilingual ability considered an asset in serving diverse communities.

Qualifications

• Bachelor or Master’s degree in Social Work or a related credential from an accredited college or university
• At least 1 year of experience in case management, counseling and advocacy services, including internship experience
• Strong interpersonal, organizational, written, and verbal communication skills
• Ability to accommodate a flexible work schedule
• Bilingual a plus

Responsibilities

• Visit families in their homes, communities or at the agency as dictated by the family’s circumstance and preventive mandates
• Engage the family using the Solution Based Casework Model
• Document all case activity in NYS Connections and NYC PROMIS
• Advocate for clients to secure services such as public assistance, health insurance and immigration issues
• Coordinate and maintain contact with ACS workers and other service providers
• Participate in Family Team Conferences, Case Conferences, meetings and trainings
• Assist in emergency situations and ensure compliance with agency policies and regulations
