SCO Family of Services in New York is seeking a Preventive Case Planner – Family Support to provide supportive counseling, assessment and case management that promotes wellbeing using the Family Connections model.

You will work with a supervisor and FC consultants, participate in FC training, and complete timely documentation, home visits, referrals and collaboration with ACS, schools and partners to reduce risk and prevent foster care.

Formación

• BA or BSW with experience.
• Evening hours required.
• Excellent organizational, written and oral skills.
• Proficient in Microsoft Office.
• Commitment to working from a strength-based perspective.

Responsabilidades

• Provide assessment, goal setting, interventions and case management.
• Learn and apply Family Connections model through training and supervision.
• Adhere to FC fidelity regarding contacts, interventions, supervision and documentation.
• Conduct regular home and office visits and accompany clients to appointments.
• Provide advocacy, outreach and referrals.
• Assess safety and risk; implement crisis and safety plans.
• Coordinate conferences with families and partners using FTC model.
• Collaborate with ACS, schools and collateral providers.
• Plan and run groups and social activities for clients.
• Complete documentation including assessments, progress notes and closing summaries.
