Job summary

United Activities Unlimited, Inc. seeks a Case Planner to manage a caseload of 12–15 families, conduct home visits, and assess safety and well-being needs. You will develop individualized service plans and coordinate collaborative planning conferences every six months to reduce risk.

The role requires a Bachelor’s in Social Work or related field, plus at least 1 year of related experience. Bilingual ability is preferred and strong communication skills are essential.

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• Minimum of 1-year case management, assessment, counseling or related work experience.
• Bi-lingual preferred.
• Excellent communication skills, both verbal and written.

Responsibilities

• Conduct safety and risk assessments for families at risk of losing their children.
• Maintain a caseload of 12–15 families.
• Create individualized service plans for each family to reduce risk.
• Facilitate collaborative service planning conferences every 6 months per family.
• Provide casework counseling and connect families to community resources and supports.
• Maintain monthly in-home visits and timely documentation.
